The Multistrada V4 is built around an aluminum monocoque frame with a 19-inch front wheel and a 17-inch rear wheel. The bike can ride on both alloys as well as spoke wheels with a wheelbase of 62 inches. Enhancing its off-roading capability, it rides 8.66 inches above the ground. At the front, the bike has a 0.50 mm fully adjustable fork. At the rear, the suspension is featured with a fully adjustable mono-shock along with remote spring preload adjustment and a double-sided aluminum swingarm. The top-tier Multistrada V4 S Sport comes with a one-of-a-kind color scheme, an Akrapovic exhaust, and a carbon-fiber front fender. The Multistrada V4 is powered by a new liquid-cooled 1158cc 90 degree V4 Granturismo engine with 4-valves per cylinder and counter-rotating crankshafts. An electronic fuel injection system supplies the fuel with 0.46mm elliptical throttle bodies. The bike features a ride-by-wire design and 6-speed transmission. Unlike other Ducati models, the Multistrada V4 doesn’t have the company's Desmodromic valves. Instead, it is equipped with a spring valve return system, which is the reason behind its exceptionally long service interval.

The shift from a Desmodromic valve to a spring valve return system makes the bike more agile for a long, service-free term. The company has claimed that a maintenance valve check is required after a long 60,000 km run. The highly modified engine is capable of producing 170 hp and 92 lb-ft of torque. The V4 Granturismo engine, weighing 147 lbs, is 2.6 lbs lighter than the previous Multistrada 1260's Testastretta twin-cylinder engine. In addition, it is also 85mm shorter, 95mm lower, and only 20mm wider. The bike is also equipped with a cylinder deactivation system that deactivates the rear bank while at the idle position to help reduce the heat felt by the rider and pillion.
